Description
Does your child have an IEP, but hasn’t been re-evaluated in over a year? Is your child with a disability experiencing new challenges and need the school to perform a re-evaluation for special education purposes? A school is required by IDEA to re-evaluate every child eligible for special education and related services at least every 3 years, but no more than once a year. This form will make a written request to the school to comply with that law, especially if they are delaying or refusing to conduct re-evaluations or only will do a ‘triennial’ re-evaluation despite observing new issues involving your child.
