Description

Does your child have a disability (or you suspect he/she does) and need the school to do an initial evaluation for special education purposes?  A school is required by IDEA to do ‘Child Find’, which means it must locate and evaluate every child in its district that has a disability.  This form will make a written request to the school to comply with that law, especially if they are delaying or refusing to conduct evaluations.
