Parents of a child with a disability may ask for an IEP meeting at any time and as often as you feel it is necessary.  You don’t have to wait for the ‘annual review’ meeting, especially if you believe the current IEP is not meeting your child’s needs.  This form can assist you in requesting a new meeting of the IEP Team (which includes you as parents/guardians).
